For your convenience, we have multiple ways to help us get acquainted with you.

1. Fill out the form online (CLICK HERE TO GO TO THE FORM). Once the form is completed you can submit it securely online, or either print it and bring it to our office. (select your choice at the bottom of the form).
   
   Firefox and Internet Explorer Users: You can submit the form electronically or print a copy to bring with you to the office.
   
   Apple or Chrome Users: Electronic submission requires Adobe Acrobat. If you would prefer not to use Adobe Acrobat then you can complete the form, print it, and bring it with you to the office. For electronic submission: download the forms to your computer, open the file using Adobe Acrobat, complete the form and then click "Submit". If you do not have Acrobat you can download it here.
2. Fill out the form at our office (do nothing now).
